Vacancies: 4 x FTE

Contract: Permanent 37hours per week

Pay: Starting at flat rate £31,537 per annum, £16.35 per hour, incremental for 5 years to £34,434 per annum, £17.85 per hour. [with yearly cost of living pay rises]

Excluding enhanced rates at weekend and bank holidays. Weekend working is factored into the working rota.

Mileage per mile is.

46.9p under 1000cc / or electric car

52.2p over 1000cc.

About the Role:
Proactive and Quick-Thinking? Join Us as a Reablement Worker! In this vital role, you will Reablement is a community‑based, short‑term service working in partnership with our integrated health colleagues. In this vital supervisory role, you will lead a team of Reablement Workers to support individuals to regain independence or determine their long‑term care needs. Working closely with Social Care and NHS teams, you will deliver support to people in their own homes—facilitating hospital discharges, or responding as part of our Prevent, Reduce, Enable rapid response service to help avoid unnecessary hospital admissions through same‑day crisis support.

You will empower individuals to achieve their highest possible level of functioning, promoting independence, choice and control. Using a person‑centred and holistic approach, you will:

Complete risk assessments and moving and handling assessments

Act as a Trusted Assessor to prescribe low‑level OT equipment

Carry out basic health monitoring (e.g., blood pressure and temperature checks)

Support low‑level healthcare tasks and rehabilitation exercises

Core service hours: 8am–10pm (service runs 7am–11pm)

• Shifts comprise of mornings, afternoons and evenings which include travel time.

• Alternate weekends paid at time and a half

• Bank holidays paid at time and a half (double pay on Christmas Day & Boxing Day)

• Mileage paid at 52.2p per mile (over 1000cc) or 46.9p (under 999cc/electric)

• Uniform, DBS, and full training provided

• A clean driving licence with Business Use car insurance required
