Summary
To provide highly specialist respiratory management to patients with Chronic Respiratory Disease and ensure a seamless integrated respiratory service between the acute and community setting. To contribute to admission avoidance, acute inpatient assessment & review, early supported discharge, outpatient clinics, community MDT clinics, Virtual MDT clinics, community home assessments and home oxygen assessment & review. To work within the multi-disciplinary team providing a holistic approach to expert respiratory care, delivering high impact respiratory interventions across the pathway.
