Summary

Shropdoc is a not-for-profit company established in 1996. We provide 24/7 urgent primary care services for patients across Powys, in addition to providing an Oncology and Haematology service for patients receiving treatment from the Royal Shrewsbury and Telford Hospitals, and also across Lancashire.

The Nurse Practitioner will deliver advanced, autonomous and person-centred care within a 24/7 integrated triage and care coordination service. The role spans both in-hours and out-of-hours provision and operates across three key workstreams: the Oncology and Haematology Helpline; Powys Care Coordination Centre (CCC), including the Single Point of Access (SPA); and Integrated Urgent Care (IUC) services for Powys.

The postholder will deliver expert clinical assessment, care planning, and make independent clinical decisions to ensure the best possible outcomes for patients and referring clinicians. Working within an extended scope of practice, the Nurse Practitioner will act as a senior clinical anchor across acute, primary, community and voluntary sector services, with a particular focus on supporting people living with cancer, individuals with complex needs, and those requiring urgent care navigation.
