Band 6 Clinical Nurse Specialist – Cardiovascular Rehabilitation

An exciting opportunity has arisen to join our friendly, passionate and nationally accredited multi-disciplinary Cardiovascular Rehabilitation Service at University Hospitals Coventry and Warwickshire (UHCW) NHS Trust.

We are seeking a highly motivatedBand 6 Clinical Nurse Specialist (CNS)to support the delivery and development of high-quality cardiovascular rehabilitation services across Coventry and Rugby. This is afull-time, substantive postworking across the Centre for Exercise and Health (Coventry), St Cross Hospital (Rugby) and University Hospital Coventry.

The post holder will play a key role in deliveringmulti-morbidity rehabilitation services, with a focus oncardiovascular disease prevention, secondary prevention and heart failure rehabilitation. You will manage a clinical caseload, contribute to comprehensive assessment and personalised care planning, and work closely with the multi-disciplinary team to support patients following cardiac events, interventions or surgery, and those at high cardiovascular risk.

You will also contribute to service development, innovation and quality improvement, ensuring evidence-based practice and national guidance are embedded into care delivery.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
• Deliver specialist nursing input to patients enrolled in cardiovascular and heart failure rehabilitation programmes
• Support CVD prevention and secondary prevention through risk factor assessment, lifestyle modification, education, and behaviour change interventions
• Provide specialist support to patients with heart failure, including symptom management, education, and optimisation of self-care
• Contribute to MDT working, case discussions, and care planning across hospital and community settings
• Support service development, audit, and quality improvement activities
• Act as a resource and role model for junior staff and students

• Registered Nurse (Adult) with NMC registration
• Significant experience in cardiovascular care, rehabilitation, heart failure, or a related specialty
