Summary

Description

We are looking for a Team Leader to work with a 10-year-old and their family. The role includes undertaking the day-to-day management of the support package. Working with their MDT and school to ensure that their needs are met and to maximise their potential. Supporting them in their home and the community.
The salary for this role is
• Weekdays - £25 per hour
• Weekends - £27 per hour
• Sleep-in - £14 per hour

Main Duties Tasks & Overview of Responsibilities
Your duties will include:

• To work flexibly as required to the needs of the client and their family.
• To liaise effectively with the family and the Case Manager regarding the rota to ensure consistent support is provided.
• To develop and maintain effective communication with the client, family and therapists.
• To follow support plans and risk assessments.
• To keep accurate daily records of each shift completed and to complete appropriate documentation as requested by the Case Manager and/or MDT members.
• To complete accurate money recording, handover notes and file notes as requested by the Case Manager.
• To communicate any concerns (accidents and incidents) promptly and document them accordingly.
• To check, complete and send in timesheets on a monthly basis to the Case Manager.
• To attend and participate in relevant training sessions.
• To monitor staff training and book on relevant courses.
• To attend meetings when required to do so by the Case Manager and / or family. To complete supervisions of the support team.
• To update support plans and risk assessments as requested by the Case Manager.
• To manage all aspects of client medication, ensuring the client has sufficient supplies that are stored and recorded correctly.
• To ensure the health and wellbeing of the client by liaising closely with the client/family in accordance with the support plan.
• To ensure health and safety of the client at home and out in the community.
• To always maintain client and family privacy and confidentiality.
• To carry out any other reasonable duties as dictated by the needs of the client.

Key skills, knowledge and experience
Essential
• Previous experience of Team Leader role.
• Previous experience of supporting clients with cerebral palsy.
• Experience in working within an MDT.
• Experience in working within an MDT.
• Experience of following MDT therapy / treatment plans.
• Experience of working in partnership with other agencies.
• Experience of PEG management (feeds, medication, care)
• Experience of devising rotas.
• Experience of undertaking staff supervisions / appraisals.
• Experience of devising and implementing documentation to assist with the management of the client.
• NVQ/Diploma Level 2/3 in Health and Social Care or equivalent e.g., completed year 1 / Level 4 of a health profession degree.
• Computer skills.
• Current certificate of administration of medication.
• Good interpersonal skills, ability to gain rapport with others.
• Effective oral and written communication skills.
• Experience of working alone, as part of a team and the ability to use initiative.
• The ability to recognise one’s own limitations and seek support if necessary.
• Confidentiality and respect of the family circumstances.
• Able to comply with moving and handling risk assessments and good general health to comply with the demands of the post.
• Car driver with full driving licence.
• Able to drive a wheelchair adapted vehicle (WAV).
• Available to work flexible shifts, including evenings, weekends, and bank holidays and to undertake sleep -in / waking night duties.
• Practical ability and skills in a range of social and domestic tasks.
• Punctual, Flexible, Patient, Trustworthy, Reliable & Understanding.
