Crynodeb
Part time 4.5 hour shifts each (3 x 4.5 hours per week shifts). Hours offered to be confirmed as this depends on interviews outcome.
The service operates seven days a week between the hours of 16:30 -00:30.Healthcare Support Worker work either between18:00 - 22:30 or between 19:30 -00:00 shifts on an alternate pro rata basis, depending on the service needs. Contracted hours up to 13.5 hours per week.
Please note the successful candidate will be based primarily at our Uxbridge head quarters in West London.
The distance Healthcare Support Worker travel each day is considerable and Band 3 Healthcare Support Worker will need to have a current UK driving licence and access to a car for work. Band 3 Healthcare Support Worker play an important role in providing seamless care and often liaise between more specialist services.
Band 3 Healthcare Support Worker work closely with people who use our Services, their families, friends and carers and play an important contribution in how people experience our services. Band 3 Healthcare Support Worker are expected to be kind and responsive but professional and informative and contribute to the quality of the services we provide by:
• keeping the people who using our services as safe as possible by recognising the limits of the Healthcare Support Worker role and working closely with Registered Nurses
• contributing to the best possible clinical outcomes by using up-to-date skills and adhering to evidence based policies and procedures when implementing care plans
• ensuring the people using our services have a good experience by respecting, empowering and working in partnership with people when contributing to the implementation and evaluation of care plans
• Carries out care following an assessment by a nurse or therapist.
• Follows a prescribed plan of care
• Performs tests unsupervised but at the request of Qualified clinician, according to manufacturing instructions and local guidelines
• Documents and reports the results according to local guidelines and communicate results to registered nurse
• Documents and reports any abnormal findings in observations
• Maintains accurate, legible, detailed and comprehensive records in the patients own home and elsewhere and report actions taken to the Senior Clinician or to an appropriate person.

The Hillingdon Twilight Service provides out of hours nursing care to people living in the Borough of Hillingdon. The types of service provided include but are not exhaustive; insulin administration, catheter care and end of life support. Staff visit patients in pairs and will be required to drive.
The post holder will work as a Healthcare Support Worker within the Twilight Team delivering care to people in their own homes and/or designated buildings. S/he will assist the qualified nurses in joint visits to carry out delegated aspects of the patients care. Contributing to the provision of a high quality Services.
