Summary

To manage referral pathway for Multiple Complex Needs including Mental Health. To chair partnership team for Multiple Complex Needs including direct partners Adult Social Care (ASC) and VIA substance misuse service. To complete and facilitate Mental Health assessments, risk assessments and complex needs assessment with individuals referred to service. To work with Housing, Adult Social Care, VIA and other relevant partners to consider complex needs, risk assessments, access requirements and referrals required to mainstream services. To work with senior management, commissioners and partners to implement NICE guidelines NG214 Integrated Health and Social Care for people experiencing homelessness within the Trust, the team and wider services. To develop a method and effective measurement of complexity to reflect needs of the cohort. To work in partnership with Commissioners and Partner agency senior management to monitor & develop the service. To complete KPI returns. To develop effective methods of coproduction with experts by experience with the relevant knowledge. To consider issues of inequality, disability access, capacity, disadvantages met by this cohort and look at creative solutions. To challenge discriminatory practices and barriers to services for this disadvantaged cohort. To support partners in considering specific needs and support requirements that will contribute to obtaining and maintaining housing. To supervise GHC, GCC & VIA staff members inc. Nursing, Recovery Workers, Social Care Practitioners, OT and Admin. To facilitate supervision of Psychology staff via Psychology team support structures. To profile current cohort in terms of needs and contribute to data collection regarding, Mental Health, Complex needs and access requirements. To provide information, advice and guidance regarding Housing & Homelessness to Trust colleagues and support the development of appropriate training. To develop links with Housing colleagues inc District Councils, General needs Supported Accommodation providers, Housing Associations and provide support and advice regarding Mental Health services.
