Responsibilities
Build trusting relationships with residents to encourage open communication and engagement.
Assess resident needs and develop personalized support plans to address identified issues.
Facilitate access to community resources and services, ensuring residents are aware of available options.
Collaborate with external agencies and service providers to promote comprehensive support for residents.
Organize and facilitate workshops, programs, and events aimed at community building and engagement.
Maintain accurate documentation of resident interactions, assessments, and services provided.
Advocate on behalf of residents to ensure their needs and rights are recognized and met.
Requirements
Bachelor's degree in social work, psychology, or a related field; relevant experience may substitute for education.
Strong interpersonal and communication skills, with the ability to engage effectively with diverse populations.
Experience in case management or social services, with a focus on community engagement.
Proficient in using case management software and Microsoft Office Suite.
Demonstrated ability to work collaboratively within a team environment and with external partners.
Excellent problem-solving skills and the ability to think critically under pressure.
Compassionate and empathetic approach to working with individuals facing various challenges.
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:00
