Summary
Provide a range of structured therapeutic interventions and clinical monitoring in accordance with National guidelines and best-practice within the wider community, working in group or on a one to one basis. Act as a key worker to service users accepted into the service and identified to be of low to moderate complexity, under the supervision of a Registered Professional. Provide support and advice and liaise/engage with partnership agencies, multi-disciplinary teams and the wider community where appropriate. Act as co-facilitator for the groups programme. Contribute positively to the achievement of service targets and outcomes. Use techniques derived from one or more therapeutic models to engage and optimise service user treatment outcomes and to support their long-term care plan. When indicated refer or signpost on to a range of other specialist and third sector services Work closely with, support and provide information to service user families/carers, and wider community services that support the service-user care-pathway. Comply with and carry out safe practice in accordance with Trust policies and procedures, appropriate Codes of Conduct, the Mental Health Act (1983) and other relevant national and local guidance. Participate in the provision of an extended hours service. Work flexibly across the county to support service need.
