Summary


The Older Adult Community Mental Health Team (OACMHT) would like to welcome a highly motivated practitioner who is passionate about advocating for Older Adults. The practitioner would be expected to be adaptable as well as creative. Also to have the ability to work autonomously and as part of the multi-disciplinary team. The team will provide intervention based care to service users who present with complex mental health needs. The team cover Preston Central, Chorley and South Ribble area.

This post is based at Charnley Fold, Bamber Bridge and will require Monday to Friday working, flexible working can be discussed prior to appointment to suit the service needs and employee preferences.

The successful applicant will have the ability to work with service users who have severe and enduring mental health issues, significant functional / psychological mental health needs, Dementia and comorbid physical health issues. We work with families and carers alongside other agencies to promote well-being and deliver evidence-based interventions.
The successful applicant will be open to new ways of working, foster and promote recovery based care and work alongside the community transformation.

Applicants will need to demonstrate their competency and experience in line with the person specification and willing to develop their skills.
