Summary
Undertake acute and non-urgent clinical consultations, both face-to-face and via telephone. Carry out same-day acute triage using the clinical system during contracted hours [08:00 to 18:30]. Process Electronic Prescription Service [EPS] requests accurately and efficiently. Review and action Docman tasks, including hospital letters and other clinical correspondence. Review pathology and blood test results, taking appropriate clinical action. Conduct structured medication reviews:- Face-to-face- Via telephone- For housebound patients- For patients in nursing and residential care homes Regularly review prescribing data to ensure clinical appropriateness and cost-effectiveness. Prescribe in accordance withGMMMG and local prescribing guidelines. Support clinical audits related to prescribing and medicines management. Plan and deliver seasonal flu vaccination clinics.
