Critical Care Outreach Nurse
We are looking for an experienced Critical Care Nurse to join our Outreach Team, providing expert care, advice, and support to patients at risk of deterioration or those who are critically ill — as well as their families and carers.
In this pivotal role, you will support patients until they are either transferred to the Critical Care Unit, their condition improves, or it is agreed that continuing this level of intervention is no longer in their best interests.
You will play a key role in developing and maintaining the highest standards within the Critical Care Outreach (CCOT) service, acting as a role model and supporting the education and development of more junior colleagues. With your experience and expertise, you will also serve as a specialist resource, offering up-to-date clinical knowledge and guidance to the wider multi-disciplinary team (MDT).

To act as a skilled member of the Critical Care Outreach team providing early intervention with appropriate assessment, observation and treatment of seriously ill patients in general ward areas i.e. level 1 and level 2 patients.
Act as a source of expert advice and support to those caring for the deteriorating patient / critically ill patients and their families on the wards.
Work as part of a multi-disciplinary critical care team providing as a point of contact for the critically ill patients. To make clinical judgements, work autonomously managing own workload, seeking advice and guidance from senior Critical Care nursing and medical staff as necessary.
Promote the nursing contribution to both the Directorate’s and Trust’s clinical governance plans.
Undertake projects, audit, and research to improve deteriorating patient/high dependency care skills trust wide.
Provide multi-disciplinary education in the deteriorating/high dependency care patient trust wide including the development of doctors in training and qualified nursing staff.
To undertake trust agreed advanced specialist clinical practice skills to meet patient needs.
Assist with the development of the Critical Care Outreach Team (CCOT)
