CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

An exciting opportunity has arisen within our in-patient Intermediate care general rehabilitation service at West Hertfordshire. We are looking for a band 3 Rehab Support Worker to join the therapy team at St.Peter’s Ward based at Hemel Hempstead Hospital

This is a service for the rehabilitation of adults presenting with a range of physical dysfunction/or disabilities resulting from injury, illness, surgery or chronic disease. The majority of the patients are referred from an acute hospital and some may be referred from the community settings and establishments.

We are looking for a motivated individual to join the team, who has the passion and skills for supporting the therapy team in the delivery of a high standard of rehabilitation. You will be working with a multidisciplinary team that includes Physiotherapists, Occupational Therapists, Nurses, Dietitian, other Rehabilitation Assistants, Speech and Language therapists and Social Workers.

• You will be responsible for a caseload of patients working to agreed treatment objectives and to assist qualified staff in carrying out individual and group treatment programs.
• Plus providing therapy and nursing interventions as part of a multidisciplinary team to patient within an agreed scope of practice.
• Successful completion of core competencies will be assessed by qualified staff.
• You will also have responsibility for handing over patient progress and any areas of concern following intervention to the relevant member of ward staff.
• The Rehabilitation Support Worker’s role will focus on improving independence and will include promotion of daily living skills, personal and social care.
