Summary
-To provide Therapy assessment and treatment of patients referred to the service designated by a qualified Occupational Therapist/ Physiotherapist or nurse - Assesses and treats patients without direct supervision according to team/department standards and protocols -Ability to manage own caseload/prioritization -To act in a mentorship role for band 3 CRAs for Intermediate care -To complete patient new take on to Intermediate care as a delegated take once the referral has been triaged by a registered healthcare professional -To complete patient allocation of visits for Intermediate care - May be a rotatory and flexible post working across Intermediate care service and our Urgent community response service .The main role will sit in intermediate care (D2A) - Working flexibly within a 7day service. - Please note: Having a driving license and a reliable vehicle might be advisable as you will be required to visit patients in their own homes.
