SLT qualification (degree level or equivalent, recognised by the Royal College of Speech and Language Therapists) and licence to practice as a SLT evidenced by registration withthe Healthand Care Professions Council (HCPC) Tobe the named SLT lead forCNRTandtodevise and implementappropriate clinicalguidelines and patient pathways associated with this area. TodemonstrateSpecialist Knowledge in clinical specialism, e.g. working knowledge of cognitive neuropsychology in language disorders, underpinned by currentevidence basedpractice. To perform advanced therapeutic assessment of patients with diverse presentations andcomplex communication and swallowing problems, to provide a diagnosis and develop anddeliver an individualised treatment programme. Tobe responsible foryour own caseload and a defined area of theservice, andprovidesupport/supervision/training to colleagues working in your specialist area. Supervision takesthe form of regular formal training and clinical reasoning sessions, peerreviewand caseconferences. To undertake all aspects of clinical duties as an autonomous practitioner. Supervise,educateand assess the performance of students (this would be to a graduatestandard and involve working with universities to ensure the standard of practice and teachingmeets the standards set by the degree level qualification). Undertake evidence-based audit and research projects to further own andteamsclinicalpractice within each rotation. Make recommendations to clinical team leader for changes topractice. May lead the implementation of specific changes to practice or contribute to service guidelines. Evidence of a commitment to continuing postgraduate professional development (CPD) through a range of activities (individual study, attendance at short courses etc.).Post holder to collect evidence of CPD by use of a professional portfolio and participation in thetrustspersonal development review process. COMMUNCIATIONS To communicate to clients and their families/carers specialist information about complex speech,communicationand swallowing difficulties, to ensure they understand and consent to assessment procedures and canparticipatefully in decision makingregardingmanagement. To use tact and empathy when dealing with sensitive issues surrounding patient care, e.g. when helping patient come to terms with the fact that his/her communication will be permanently impaired. To apply highly developed listening and visual observation skills with clients and others in a range of settings, e.g. during structured one-to-one sessions, home visits, working both with direct observation and using audio and video recordings.This will include tasks such as making phonetic and linguistic transcriptions, analysing speech and voice patterns, observations of social dynamics and non-verbal communication. Assessment of swallowing will require fine motor skills for palpation (feeling of laryngeal activity during swallowing). To contribute tothemulti-disciplinaryteam indiscussion of own and others input aroundclients needs and management. Use a variety of communication methods throughout the working day,eg: email, writtenrecordsand telephone. Ensure that all locally produced information documents are in the corporate style and that all written informationcomplies withTrust policy. PLANNING AND ORGANISING To organise own workload effectively, prioritising tasks andplanning aheadby setting both long- and short-termSMARTgoals.To plan and oversee work carried out by newly qualified SLTs,Technical Instructorsand students. Todemonstratereflective practice, reflecting on practice both independently and with peers; todemonstrateproblem-solving skills in a peer-review/ support context andidentifyown areas of developmental needs. To ensure attendance at all mandatory training and to attend regular staff meetings. RESPONSIBILITIES To manage a specialist caseload of adult clients including patients with communication,swallowand voice difficulties. Carry full responsibility for assessment, diagnosis,treatmentand discharge. To assess and diagnose all in andout patientsin general and own specialist caseloads. To work collaboratively with the clients families/carers and other professionals, during assessment and allsubsequentmanagement, including decision making. To communicate complex condition-related information from assessment to clients and their families/carers; this may include giving unwelcome or difficult to accept information e.g. around the seriousness,implicationsor long-term nature of the difficulty. Toprovidespecialist advice to patients, carers, nursing staff, Allied Health Professional (AHPs), medicalstaffand othersregardingthe management/treatment of patients with communication and/or swallowing difficulties. To work in conjunction with team leaders butdemonstratingskills toidentifyand deliver care plans forhighly complexcases andgenerate and implementappropriate strategiesfor caseload management. To provide written reports, presenting complex information in a clear and accessible way; reports may be provided to carers, GPs, medicalconsultantsand other allied health professionals. To direct clients/carers toappropriate voluntaryagencies and external services for support and information, e.g. The Stroke Association and other charitable groups. Toparticipatein the delivery and development of specialisttraining in many settings (e.gNursing Home,CNRT department) to nursing staff, AHPs, carers.Training will cover communication,dysphagiaor both. To take a lead role in developing and delivering specialist training (formal and informal) to othersin area of clinicalexpertise. To work with Technical Instructors/Speech and Language Therapy Assistant, providing programmes of therapy to be used with individual patients and supervising their implementation. To provide training for staff within the department.To provide clinical training and supervision in own specialist areas to SLT students and junior/specialistSLTs, other professionals, SLT Assistants and other students, e.g. student nurses. To act as mentor forspecialist SLT andjuniorstaff; to provide advice and support to assistants and volunteers. Tomaintaincomprehensive,accurateand up-to-date case notes for own clients, adhering to professional andtruststandards. To undertakeaudit anddata collection as required by thetrustandsubmittimelyandaccuratedata asrequired; this will include information about patient contacts, caseloadfiguresand activity levels. This position requires the post holder to be able to travel between diverse clinical and non-clinical environments and bases, includingpatientshomes, GP clinics, primary care facilities, hospitals, officespacesand residential/nursing homes, so access to reliable transport is essential. The post holder will be expected to complete an agreed number of clinical contacts a week, at separate addresses tomaintainprovision of our service when working within CNRT.
