Summary
We are looking for a passionate and motivated Care Coordinator to join our Connecting Care Team East.
Our team supports adults aged 18 and over who have care and support needs, working to promote independence, dignity, and wellbeing. Connecting Care Team East is a highly supportive and friendly team with a wealth of experience in adult social care.
As a Care Coordinator, you will play a key role in helping people live as independently as possible. This includes undertaking conversation-based social care assessments, re-assessments, reviews, and developing outcome-focused support plans that reflect individual needs.
You will take a proactive approach in responding to individual circumstances, signposting to appropriate services, and determining eligibility through to completion of assessments in line with the Care Act 2014. Your work will be rooted in a person-centred, strengths-based, and outcome-focused approach, ensuring that people remain at the heart of everything we do.
While a social work qualification is not essential, you must have knowledge of care management and an understanding of the Care Act 2014. It is also important that you are well-organised and have up-to-date knowledge of relevant legislation, including the Mental Capacity Act 2005 and Human Rights Act 1998.
