Summary
Patient Training and Support Provide one-to-one and small group training sessions for patients on using the practice website, NHS App, and other digital health tools Support patients with online appointment booking, prescription ordering, accessing medical records, and video consultations Adapt training approaches to meet diverse needs, including older adults, people with disabilities, and those with limited digital literacy Research simple, accessible guides and resources tailored to our patient population Community Engagement and Outreach Work collaboratively with neighbourhood organizations, community groups, and local partners to reach digitally excluded individuals. This may include, Age UK, Camden Council and the Camden Health Bus Organize community drop-in sessions and workshops in accessible local venues Build relationships with voluntary sector organizations, libraries, community centers, and faith groups Identify and address barriers to digital inclusion, including access to devices and connectivity Build links the main universities around Gower Street Practice and investigate offering Digital Inclusion sessions for patients including overseas students Digital Health Ambassador Programme Recruit, train, and support a team of volunteer Digital Health Ambassadors from within the community Develop training materials and resources for ambassadors to use Coordinate and supervise ambassador activities, ensuring quality and consistency Create a sustainable peer-support network that extends beyond your direct reach Promotion and Communication Actively promote the benefits of the NHS App and digital health services Develop engaging communications materials for diverse audiences Monitor uptake of digital services and identify target groups for focused support Share success stories and case studies to inspire confidence in digital tools Monitoring and Development Track participation, patient feedback, and digital service uptake Provide regular reports to the practice team on progress and impact Stay informed about developments in digital health services and accessibility features Continuously improve the programme based on patient feedback and evaluation
