Summary
Working in the community, the post holder will oversee the management of inpatients on both rehabilitation and subacute pathways and provide medical input to the ARC a multidisciplinary clinic completing CGAs with patients referred by GPs, South Central Ambulance Service and secondary care. Working across the two trusts they will build relationships with acute medicine, radiology, frailty and community services, which will enable them to develop new pathways to support a seamless transition of care.
