This is an exciting new post, shared between Berkshire Healthcare NHS Foundation Trust and Frimley Health NHS Foundation Trust, to develop a truly integrated way of working between the acute and community sectors.

The post holder will have the opportunity to work in the acute hospital, supporting the acute frailty service for one day a week and participating in the GIM on call rota if dual accredited or they have equivalent experience. They will maintain their skills in acute inpatient medicine whilst also providing insight into alternative models of care in the community.

Frimley Health NHS Foundation Trust – Acute Frailty

· Act as senior decision maker for the Acute Frailty Team, completing timely Comprehensive Geriatric Assessments, participating in Advanced Care Planning discussions and exploring alternatives to hospital admission where appropriate.

· Provide supervision and training to Advanced Practitioners and resident doctors rotating through the team and leadership within the multidisciplinary team.

· Participate in the GIM on call rota, reviewing patients post take in ED and the Acute Medical Unit.

· Engage with quality improvement initiatives to improve the care of older people in the acute trust.

Berkshire Healthcare NHS Foundation Trust – Community Geriatrics

· Clinical case management for patients admitted to the community wards (Henry Tudor and Jubilee) and referred to the Assessment Rehabilitation Clinic.

· Active engagement in multidisciplinary team meetings reviews in primary care, including community settings, the Assessment Rehabilitation Clinics, Virtual wards and community hospital in-patient units.

· Reduce unnecessary acute unplanned admissions, length of stay and premature/inappropriate entry to institutional care.

· Provide safe, effective and timely rehabilitation/re-enablement in the community.

Working in the community, the post holder will oversee the management of inpatients on both rehabilitation and subacute pathways and provide medical input to the ARC – a multidisciplinary clinic completing CGAs with patients referred by GPs, South Central Ambulance Service and secondary care.

Working across the two trusts they will build relationships with acute medicine, radiology, frailty and community services, which will enable them to develop new pathways to support a seamless transition of care.
