Summary
Clinical Responsibilities: To act as an independent, autonomous practitioner, using advanced clinical skills to treat and advise on clinical management of stoma therapy and advise on management of complex abdominal wounds related to fistulae. Is wholly accountable for his/her practice and takes every reasonable opportunity to sustain and improve his/her professional knowledge. To ensure good communication with all members of the team for example through attending ward rounds and MDT meetings. To develop and sustain a high quality service with trust wide multi-professional teams. To meet/visit elective patients pre-operatively to provide information and education to patients and their families regarding their proposed operation, stoma formation and to site the stoma. This care should be provided for patients undergoing emergency stoma formation where possible. To advise staff as required in developing and appropriate plan of care for each patient incorporating discharge planning. To provide samples and advice on suitable appliances and accessories as required, ensuring impartiality at all times. To provide post-operative information, counselling and support to ensure that the patient and their carers have a level of knowledge and skill commensurate with their needs. To provide follow up care and support during outpatient and home visits as appropriate. To offer a supportive service to patients with a stoma and their carers, disseminating specialist knowledge to enable individuals to try and adapt to their surgery liaising with other healthcare professionals when necessary.
