Summary


Community Mental Health Nurse

This is a community mental health role within Tower Hamlets Community 24/7 service pilot. The pilot ensures delivery of high quality, modern, patient-centered services. This is a pilot project aimed to establish and test out a new model of care that breaks down current barriers to care, is non-restrictive, co-produced, culturally sensitive and accessible. The service will run 24hrs a day, 7 days a week for people residing in the catchment area offering open access to a range of psycho-social- medical Interventions at every stage of their wellness journey, this includes short term Hospitality stays for those in crisis, and community based offers that are embedded in the neighbourhood.





The role involves, the successful candidate fulfilling the below



· Welcomeall attending the community hub within its values and ethos

· Make sure that co-production is at the core of the role

· Ensuring that compassion and empathy are central to care delivery

· Ensuring that the local demographics are taken into consideration for service provision andimprovement

· Participating in all MDTworking eg meetings, visits, assessments and interventions

· Making Contact with the service user in a timely Manner following from Triage to screen forrisk effectively and to complete initial assessment.

· Toberesponsibleforanidentifiedcommunitycaseload,ensuringhighstandardof professional conduct to comply with best practice and professional accountabilities.

· Topromoteaserviceuserfocusedservice.

· Participateinauditsandsurveyrelevanttotheteam.

· Toparticipateindevelopmentofnewpoliciesandproceduresasrequired.

· Ensurethatclientsreceiveaculturallycompetentservice.

· Toco-ordinatetheday-to-dayactivityandmanagetheworkloadoftheTeam.

· Toensurean in depth understanding of the Mental Health Act (1983); Care Act (2014); Deprivation of Liberty Safeguards (DOLS)

· To establish and maintain good relationships with service users, carers and voluntary sectorcolleagues.
