If you are a self-motivated, compassionate and resilient individual who want to play a role in transforming the lives of some of the most vulnerable people in society, we want to hear from you. As a Rough Sleeper/MEAM Coordinator you will be working with those people in the locality who are rough sleeping, at risk of rough sleeping or considered at risk of facing exclusion from services.
You will assist in developing a Making Every Adult Matter (MEAM) Pilot for Harrogate, working as part of a multi-disciplinary outreach team which includes Adult Social Care, Drug and Alcohol Support Services, Housing, TEWV and lived experience. You will carry a caseload of up to 15 people with multiple needs (some of which will be entrenched rough sleepers).
You will provide a holistic and coordinated support for all customers, from first contact in a street setting, through to them achieving a more settled and fulfilled lifestyle. Working alongside the council’s Housing Options team and Community Safety HUB.
The role will involve you working in a multi-agency setting, co-ordinating services on behalf of this customer group.
The ideal candidates will have the ability to work using a supportive person-centred approach using a person centred trauma informed approach, with individuals who have been rough sleeping and the trauma associated with that. You will also have experience of working with people with multiple needs, knowledge of the current welfare system, private rented sector and understand the barriers to engagement.
Additionally, you will be able to work flexibly and creatively according to the needs and patterns of the cohort, which will entail street-based work alongside other services.
