Summary

Job responsibilities The Enhanced Access service operates on behalf of Hambleton and Richmondshire PCNs and is delivered from a number of local General Practices and hospital. The role is to cover a variety of shifts which are convenient to you. We offer in hours shifts (14.00-18.00hrs), and shifts on an evening (18.30 - 20.00hrs) and weekends (Sat 09.00-17.00hrs & Sun 09.00-13.00hrs). We have many clinical services which require the expertise of a Practice Nurse to complete the service delivery team. We deliver Diabetes education programme, Respiratory Diagnostics Service and our Enhanced Access Service. Clinical Tasks Clinical measurements including blood pressure recording, blood glucose monitoring and ECGs Perform cervical cytology screening. Undertake wound care and appropriate management of wounds. Undertake long term condition reviews and record via Ardens templates Prepare, maintain, and restock treatment areas and equipment before and after patient appointments. Participate in health promotion and patient education activities. Maintain cleanliness and hygiene to infection control standards Escalate clinical concerns to a senior nurse or GP promptly. Communication Communicate sensitively and effectively with patients, carers, and team members. Adapt communication methods to suit patients' individual needs, backgrounds, and understanding and perspectives. Maintain open, respectful, and professional communication within the team and with external partners. Represent patients needs and perspectives when appropriate. Quality of Care Work within your level of competence and training. Work consistently within your own professional standards, maintaining an awareness of evidence based and current best practice. Manage your workload effectively, maintaining high standards of patient care. Deliver care aligned with NICE guidelines, local protocols, and evidence-based practice. Support audit cycles and use patient feedback to enhance services. Adhere strictly to confidentiality and data protection regulations. Uphold Duty of Candour and safeguarding responsibilities for vulnerable patients. Follow policies for risk management, domestic violence, and substance misuse. Health & Safety Follow the practices Health & Safety policy. Identify risks in the workplace and act to reduce them. Report accidents or incidents and ensure follow-up actions are taken.
