Summary
Main Purpose of the Role
To deliver high-quality, person-centred care and support that promotes independence, dignity, and wellbeing. You will work as part of a multidisciplinary team to assist individuals with rehabilitation goals, daily living activities, and emotional support during their recovery journey.
Key Responsibilities
1. Person-Centred Care
• Provide compassionate, respectful, and professional care to all residents.
• Support individuals with washing, dressing, toileting, continence care and personal hygiene needs.
• Assist individuals to mobilise safely using appropriate equipment and techniques.
2. Rehabilitation & Reablement
• Work alongside physiotherapists, occupational therapists, nurses and senior carers to implement personalised rehabilitation plans.
• Encourage residents to participate actively in their recovery, promoting independence with daily activities such as cooking, walking, and personal care.
• Support exercises, functional activities, and mobility routines as advised by clinical professionals.
3. Support with Daily Living
• Assist residents with meal preparation, hydration, and nutritional needs.
• Help maintain a clean, safe and homely environment, including light domestic duties.
• Support residents with practical day-to-day tasks such as laundry or accessing communal areas.
4. Monitoring & Record Keeping
• Observe and report any changes in residents’ physical or emotional wellbeing.
• Accurately complete daily notes, care plans, risk assessments, and incident reports in line with organisational and regulatory standards.
• Communicate effectively during handovers and team meetings.
5. Safety & Compliance
• Follow all safeguarding, confidentiality, and data protection procedures.
• Adhere to infection control and health & safety policies at all times.
• Use moving and handling equipment safely and correctly.
6. Teamwork & Professionalism
• Work collaboratively with colleagues, visiting professionals, families, and external agencies.
• Uphold the values and standards of White Oaks Living in all interactions.
• Participate in training, supervision and appraisal to continuously develop skills and knowledge.
