We are looking for an enthusiastic professional who can help the Community Independence Service (CIS) make Hammersmith & Fulham (H&F) a great place to grow old.

The Community Independence Service is co-located with London Borough of Hammersmith and Fulham (LBHF) Adult Social Care on King St, Hammersmith as well as within Charing Cross Hospital.

Our service works closely with our acute partners, mainly Charing Cross but also other acute Imperial College Healthcare NHS Trust sites and Chelsea & Westminster Hospital NHS Foundation Trust.

You will be connected with peers across the tri-borough of Hammersmith & Fulham, Westminster and Kensington and Chelsea to support you and to work on service improvement partnerships.

As the senior physiotherapist/Occupational therapist for the CIS Home first team, you will be expected to be a clinician with a broad range of clinical and line management competencies who is able to adapt to a wide variety of circumstances.

You will use evidence based principles, current best practice and specialist skills to assess, plan, implement and evaluate interventions and to optimise patient safety following their discharge from hospital.

You will deliver skilled physiotherapy or occupational therapy care to clients in their own home, based on a thorough holistic and risk assessment of their clinical and personal needs and the agreement of an individualised care plan.

• To prioritise factors involved in the clients presenting condition through clinical reasoning based on knowledge of current professional opinion, client’s prognosis and other physical, social, emotional and mental health issues.



• To manage the effective coordination and continuity of client care working closely with external agencies and community colleagues.

• To ensure that clinical practice is contemporary to meeting the needs of clients and families.

• To ensure that each client has a comprehensive assessment of need, including risk assessment and that their care is culturally sensitive and addresses issues of diversity.

• To work closely with the acute trust, GP, nursing and therapy colleagues to prevent inappropriate hospital admissions.

• Attend regular MDT and daily handover meetings as appropriate.

• To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care.

• To evaluate your practice through audit and outcome measures with support of senior colleagues when required.

• To maintain and develop specific departmental duties as designated by pathway lead.
