Summary
To act as a clinical expert taking a lead role in developing meaningful and achievable safe early discharge plans for patients whose risk profile and clinical presentation indicate that care and treatment no longer needs to be provided in an inpatient environment. To take an active leadership role in enabling the team to meet all relevant key performance indicators, particularly those associated with being a CRHTT that meets Home Treatment Accreditation Standards and the requirements of being a CRHTT model that meets CORE fidelity.
