Summary
We are looking for an experienced Practice Nurse to join our Practice.. The role will involve providing clinical assessment and management of chronic medical conditions with a particular focus on diabetes. As a Diabetes Nurse, you will play a key role in managing and supporting patients with diabetes, working independently as well as part of a team to deliver a high standard of patient care. Your role will involve providing comprehensive diabetes management, including education, monitoring and treatment to patients. You will also support patients with prevention strategies, lifestyle changes and initiate treatment for newly diagnosed patients. Job responsibilities Be involved in the assessment, management and treatment of chronic health conditions such as Diabetes, Asthma, COPD and Hypertension. The role will involve a particular focus on the management and support of patients with diabetes. This will involve running diabetes clinics (with GP support). Cervical screening clinics. Immunisations To undertake a range of nursing assessments and provide appropriate care and treatment in conjunction with the GPs, Nurse Practitioners and other practice nurses or independently, in line with competence and need. Assisting the Practice in meeting targets for the Quality and Outcomes Framework (QOF). Diligence in record keeping. Working as part of a team involving doctors, nurses, receptionists and treatment room staff. Maintain clinical knowledge and be motivated to pursue independent study in line with perpetually evolving clinical guidelines.
