Summary
Job Purpose To deliver high quality, patient-centered medical care to nursing home residents, ensuring proactive management of chronic conditions, timely response to acute issues, and compassionate end-of-life care. The role requires collaboration with nursing home staff and multidisciplinary teams to optimise health outcomes and maintain compliance with regulatory standards. Key Responsibilities Conduct comprehensive clinical assessments and ongoing care for nursing home residents. Diagnose, treat, and manage acute and chronic illnesses in line with best practice and local guidelines. Perform regular medication reviews and optimize prescribing for frail and elderly patients. Provide palliative and end-of-life care, including advanced care planning discussions. Liaise effectively with nursing home staff, families, and external healthcare professionals. Maintain accurate and timely clinical documentation in accordance with regulatory standards. Participate in safeguarding processes and ensure compliance with CQC and local governance requirements. Contribute to quality improvement initiatives and audits related to nursing home care.
