Summary


Under the direction of the Lead Nutrition Nurse the post holder will act as an informed resource; to provide specialist advice and support for patients with complex nutritional challenges requiring enteral and/or parenteral nutrition, and act as a role model, whilst developing competencies as a clinical expert in their field of specialism.

Work within the NMC code of professional practice, and within Calderdale and Huddersfield NHS Foundation Trust policies, procedures and guidelines.



Be an integral member of the multi-disciplinary team providing nutritional care to patients in hospital and from the community.



Deputise for Lead Nurse as appropriate.



Provide excellent, specialised, evidence based clinical nursing practice in nutrition support including nutritional screening and clinical assessment of patients.

To assess, plan and implement care to patients within specialist area. To provide and maintain high standards of care, to patients requiring specialist input and advice.



Train, guide and supervise general registered nurses/midwives, allied healthcare professional, patients, carers and relatives, in hospital to understand specialty area and aspects of care.



Assist and support the multidisciplinary team by ensuring patient’s conditions are reported accurately, that plans of care and instruction is correctly conveyed to ward nurses, medical staff and that prescribed treatments are administered.
