We are recruiting for an enthusiastic registered nurse to work as part of a small team with a health visitor to lead, deliver and influence the national Health Visitor Pathfinder Project in Croydon. The post holder will deliver a local service specification of childhood immunisations that meets the public health needs of the local communities in Croydon. The post holder will work in the North Locality of the borough covering 4 PCN's which has 24 GP Practices.
The aim of the pathfinder project is to utilise the health visiting service to increase the uptake of childhood vaccinations for children aged 0-5 years in domiciliary or alternative settings.

• To provide a skilled, effective and evidence-based immunisation service to clients in a variety of community settings.
• The post holder will work collaboratively with the health visitor to plan, organise and deliver childhood immunisations in line with the national childhood immunisation programme.
• Work collaboratively to support the team, organisation, and company stakeholders, to facilitate the smooth continuity of service delivery.
• Support, train and educate health visitors in delivery of the national immunisation programme.
• Attend sites across different locations as directed by the line manager.
• Ensure all clinical incidents, near misses and complaints are reported, monitored and processed in line with organisational policies and procedures.
• Participate in audits, making recommendations based on findings and sharing outcomes with the team for learning.
• Ensure record keeping is up to date and in line with company policy, procedure and guidelines.
• To work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities.
• To collaborate with team colleagues in order to deliver the contractual requirement of NHS England.
