Summary
Please refer to the attached job description and person specification for full details of responsibilities To plan, implement and evaluate individualised specialist patient care programmes based on evidence-based practice and the application of clinical reasoning skills. To provide supervision to health care professionals in relation to: prevent pressure ulcers/Moisture Associated Skin Damage (MASD) and other wounds, use of equipment and all aspects of managing incontinence. To be professionally and legally accountable for all aspects of your own work including management of patients in your care. To organise the job role effectively regarding clinical priorities and use of time. Act as a patient advocate through the application of critical and professional knowledge and skills. The post holder will require a high degree of communication skills in communicating highly sensitive information regarding individuals medical conditions. To ensure effective communication with the wider community and Healthcare professionals, to persuade change, influence and improve practice. Empathy, reassurance and negotiating skills are required to encourage patient compliance to treatment/management strategies, where there may be significant barriers to understanding in some patient groups e.g. learning disabilities, mental health. Provide advised to palliative care services in wound and equipment management as appropriate. Promote better understanding on prevention and management of wounds, MASD and equipment prevision to patients, their families and carers, and other agencies involved with the patient through education and support. Ensure personal clinical skills are regularly updated as required by Lead Clinician to a specialist level. Participate in research and clinical trials in accordance with Clinical Governance and Trust policy. Involvement of the service user, carers and other professional including clinical management plans where required.
