Summary
Job Summary: As part of the PCN multidisciplinary team you will be responsible for the following: Clinical Delivery The Enhanced Nurse Practitioner will serve as the Clinical Lead for the Andover Health Hub, providing expert clinical support, oversight, and guidance to the General Practice Assistants (GPAs) and Care Coordinators. In this capacity, the practitioner will facilitate safe and effective clinical decision-making within the team, ensuring high standards of patient care. In addition, the Enhanced Nurse Practitioner will act as the Clinical Lead during COVID-19 vaccination campaigns, overseeing the delivery of safe and efficient vaccination services in line with national and local guidance. A key element of this role will include supporting the frailty pathway across the PCN. The practitioner will contribute to frailty management, including undertaking Structured Medication Reviews, participating in multidisciplinary frailty reviews, and completing home visits where clinically appropriate. To support the five core network practices, the practitioner will also provide one clinical session every other week, tailored to practice needs and individual practitioner competencies. These sessions may be delivered on site within practices, from the central Health Hub, or remotely (for example, within care home settings), ensuring a flexible and responsive model of care. Education & Supervision The Enhanced Practitioner will provide clinical supervision, education, and skills development for the Health Hub team, ensuring that GPAs and Care Coordinators have the necessary clinical competencies to fulfil their roles effectively. The practitioner will also support workforce development by identifying learning opportunities and fostering a culture of continuous professional growth. Administrative Leadership & Support The practitioner will act as the Infection Control Lead for the Health Hub, ensuring compliance with infection prevention and control (IPC) policies and procedures in alignment with national and local IPC standards. Additionally, the practitioner will work in collaboration with the non-clinical Health Hub Manager to review and update relevant policies, protocols, and standard operating procedures (SOPs), ensuring they reflect current clinical best practices and regulatory requirements. In order to work at this level, you should have a. a degree in nursing or from a registered nurse degree apprenticeship (RNDA); b. registered with the Nursing & Midwifery Council c. has a post graduate qualification at level 7 or above relevant to their area of enhanced practice, for example in wound care, diabetes plus respiratory and CVD, dementia, women's health and public health and population health management, or other area of enhanced practice.; d. is working at an enhanced level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework e. works across the PCN, in services that enhance the core general practice offer. Key Responsibilities: The enhanced nurse practitioner will be working across the Andover Primary Care Network doing a combination of practice work and at the Health Hub delivering the following key responsibilities a) works as part of a MDT within the PCN; b) consults with patients, their family, and the multi-professional team to undertake assessments of patient need and devise and evaluate complex care plans. c) evaluates and analyses clinical problems using their clinical knowledge, seeking out and applying relevant evidence, enhanced techniques, interventions, and equipment to make clinical decisions. d) delivers enhanced clinical care in the context of continual change, challenging environments, different models of care delivery, innovation and rapidly evolving technologies using analysis and their underpinning knowledge to manage complex interventions. e) teaches and advises patients and their families on how to manage their condition or support the multi-disciplinary team to do so. a) participates in clinical audits and research projects and implements changes as required, including the development, and updating of practice protocols / guidelines and procedures locally. b) works within national and local protocols where these exist. c) recognises boundaries of their practice and know when and to whom patients should be referred. d) using the principles of delegation, delegates work to other members of the MDT and take responsibility for the delegated activity as appropriate. e) demonstrates initiative and are creative in finding solutions to problems. f) holds responsibility for team performance and service delivery; and g) provides supervision to trainee nurse associate, nurse associates and general practice nurses. Wider Responsibilities: 1. Support with the delivery of structured medication reviews, focusing on polypharmacy and deprescribing. 2. Facilitate and support the development and delivery of innovative training and education to staff, to improve competence and confidence, in line with best practice and guidance related to area of expertise. 3. Participate in and/or lead formal training events promoting best practice in area of expertise. 4. Provide professional and clinical leadership and mentorship, acting as a clinical role model on the delivery of evidence-based practice. 5. Advise and support on the development and delivery of work streams relating to area of expertise as part of the PCNs strategy, where appropriate. 6. Work proactively with key local and national stakeholders to develop more integrated care pathways and holistic models of care, through dissemination of specialist skills and knowledge. 7. Contribute to the delivery of local policy, procedures and protocols for the PCN to improve identification, assessment, ongoing support for patients and their family carers.
