Summary
An understanding of the continuing care framwork is required alongside experience of case management and multi-agency working is an essential requirement for the post as close collaboration takes place with community and acute services as well as education and social care. This is a community-based post and although the role is based in Birmingham and Solihull, the post holder will be expected to be able to travel covering the whole of the area and some out of area travelling may be required. We are currently continuing to work from home to support children and families. The role will be varied with the practitioner undertaking assessments, attending MDTs and delivering training to multi agency professionals
