Join Our Team as a Dementia Support Coordinator!
We’re looking for a Dementia Support Coordinator to join our Calderdale Dementia Hub team. You'll be an integrated member of a partnership between Age UK Calderdale & Kirklees.
You’ll be responsible for initial contact with clients and carers, dealing with referrals, arranging consultations and visits as appropriate.. This role will entail building positive relationships with the Age UK team, as well as other organisations within the community to provide the best care and support for those with and affected by dementia.

• Posses a knowledge & understanding of living with dementia from both the client and carer/family perspective
• Can champion a Trauma-Informed, strengths-based approach that celebrates every individual’s assets.
• Have experience of managing a client caseload and providing advice, guidance and signposting.
• Can build and nurture relationships with stakeholders, agencies, and community networks.
