We are searching for Band 4 Assistant Practitioner (AP) to come and join our respiratory nursing team in the Mid Notts Cardio Respiratory service mainly covering Mansfield and Ashfield - may be a need to work across Mid Nottinghamshire portfolio when needed...could this be you?
You will be working in a community based respiratory nursing serviceproviding specialist nursing care which includes holistic and specialist respiratory nursing care including - inhaler technique demonstration / review, completing holistic assessments and components of respiratory nursing assessment, palliative nursing care in patient’s homes and residential / nursing care homes. You will be able to plan and evaluate patients personalised care needs. You will be able to refer to other services both within the Mid Notts Cardio Respiratory Service and outside as needed.
The Assistant Practitioner is required to compliment the current respiratory nursing establishment, enhance the skill mix and become an integral member of the service
The Mid Notts Cardio Respiratory Service is part of the wider community health services team providing long-term conditions management in the following specialities; Pulmonary Rehabilitation, Heart Failure Nursing, home Oxygen Assessment Service and Respiratory Specialist Nursing.
We are looking for an assistant practitioner to join our dedicated Respiratory Nursing teams and wider cardio respiratory team who are committed and passionate about promoting health, well-being and independence. We are looking for a dedicated and enthusiastic clinician who believes in the provision of patient-centred care.
A key aim of our service is to reduce unplanned admissions and facilitate discharges from the acute care environment. The team members include , Community Nurses, District Nurses, Specialist Nurses, Support Workers and Social Workers, Acute Hospital Professionals and also working closely with GPs and the voluntary sector.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

You should possess good interpersonal and time management skills, be a good communicator, organised and able to prioritise and work well as a member of a team.
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.
