Summary


We are looking to appoint an experienced and enthusiastic physiotherapist to work with adults who have a learning disability and complex health care needs.

The CLDT has an integrated management structure and multidisciplinary and multi-agency approach to meeting client need.

The post offers opportunities for the successful applicant to develop skills in meeting the complex needs of clients who have additional physical, sensory, behavioural/mental health needs. You will be putting person centred, innovative therapeutic interventions in place to enable clients to develop and maintain independence in a community setting and prevent hospital admissions.

The clinical role will include 24-hour postural management, assessment for specialist therapeutic equipment, respiratory management, hydrotherapy and rebound therapy. Within this post you will have an opportunity to work with clients transitioning back to Bolton from out of area, and clients transitioning from school into adult services.

You will need to bring creative thinking and all skills you have acquired and developed in your Physiotherapy training and previous posts.

There is a commitment to providing ongoing training and professional development.

To undertake specialist physiotherapy assessment of clients with diverse and complex presentations.

Manage a complex caseload

To identify physiotherapy goals and develop individual client centred treatment plans.

Lead in assessment and provision of specialist equipment.

To work in collaboration with others to facilitate client access to external services.

To complete detailed comprehensive written reports.

To demonstrate communication skills, using a range of verbal and nonverbal communication tools to communicate effectively with clients with varying degrees of learning disability

To prepare and submit timely, accurate and comprehensive detailed reports and clinical guidelines.

Maintain accurate and comprehensive records in line with standards and policies.

To represent and promote the physiotherapy role within the MDT.

To take a lead in an area of service/quality improvement

Supervision of more junior staff members.

To deliver advice and training activities.

To be responsible for the clinical education of student physiotherapists on placement within the CLDT.

Participate in the review, planning and development of the Physiotherapy Service within the CLDT.

To demonstrate ongoing personal and professional development in accordance to HCPC standards.

The Community Learning Disability Team is a fully integrated MDT environment. We work closely with our social care colleagues.

As physiotherapists/AHPs within the team we work hard to meet the health needs of the learning disabled population of Bolton.

The CLDT is renowned within the FT for being a very supportive place to work. The leadership structure is clear but aims to be none hierarchical in it's approach.

Ideas for change, improving efficiency and quality improvement are actively encouraged.

The atmosphere is professional but friendly and welcoming. Informal supervision and opportunities to discuss complexities of a case are always available. There is a clear ethos of working together to get the best possible outcomes for our service user group. The individual skills and knowledge brought by each profession and clinician is highly valued.
