We are delighted to offer this recruitment opportunity for an experienced Nurse or Social worker level 2 (known internally as Qualified Practitioners Level 2) within our Louth &Horncastle Team. This is a full-time permanent position - 37 hours per week.

The Qualified Practitioner level 2 role is both an exciting and challenging role. For those that love fast paced and ‘make a difference’ work, this is the position for you. The Learning Disability Team is proud of the Practitioners we have in our team. They take great pride in doing their job well and are passionate about providing the best service they can for the people who access our specialist service.

About the Role

You will have an allocated case load which includes complex cases. Working in a Person-Centred way, carrying out strength based Adult Needs Assessments and Reviews focusing on what the person ‘can do’ rather than what they ‘cannot do’. Working with the people we support and their families to maximise independence.

Our aim is to support people to reach their goals and aspirations. Whether that be employment, community access or moving into their own home. We help people to overcome the obstacles and challenges they may face, supporting them to reach their potential.

You must have use of a vehicle, be able to travel effectively and within a reasonable timescale to meet service requirements, this could include being responsive to last minute unplanned visits which may be non local/out of county appointments.

As well as supporting our customers you will supervise up to three non-qualified team members, supporting their development and professional practices. Completing observation of practice, Audits and case support.

Our core office hours are 08:45–17:15 (Mon-Thu), 08:45–16:45 (Fri). We currently work a mixture of home based and office working, however there will be a requirement to be in the office when required to meet the needs of the service.

About you

We are proud of our teams and the service we provide; it is important that we recruit the right person to ensure we can give the best support possible to our teams and our service users.

The ideal candidate will need to hold either a Social Work or Nurse qualification and be able to show that their registration with SWE/NMC is in date. The nurse qualification does not need to be specific to Learning Disability.
