Seeking a highly motivated and experienced Children's Care Coordinator to join the integrated health and social care team. If you are passionate about holistic, person-centered care and have a proven track record of coordinating complex support packages for children and young people (0-18) and their families, we want to hear from you.
About the Role
As an experienced Care Coordinator, you will act as a central point of contact and advocate for a diverse caseload of children with complex health, developmental, or social care needs. Your expertise will be crucial in navigating multiple agencies and ensuring seamless, effective, and person-centred support.
Key Responsibilities:
• Case Management & Coordination: Proactively manage a caseload, conducting comprehensive, strength-based assessments of children's and families' needs.
• Care Planning: Develop, implement, and rigorously review Personalised Care and Support Plans in collaboration with the child, family, and multidisciplinary team (MDT).
• MDT Liaison: Facilitate and lead meetings with various professionals (e.g., GPs, social workers, therapists, educators) to ensure a cohesive and coordinated approach to care.
• Advocacy & Navigation: Act as a strong advocate for the child and family, helping them navigate complex health, educational, and social care systems and accessing appropriate resources (e.g., funding, equipment, specialist services).
• Record Keeping: Maintain accurate, up-to-date, and confidential electronic records in compliance with all relevant safeguarding and data protection policies.
• Safeguarding: Uphold the highest standards of child protection, recognizing and responding appropriately to all safeguarding concerns.
What You Will Bring (Essential Criteria):
• Education: A relevant professional qualification in Health or Social Care (e.g., Social Work, Nursing, Allied Health Professional) or a Level 3/4 Diploma in Health & Social Care/Children's Services, or equivalent experience.
• Experience: Minimum [3-5] years of demonstrable experience in a Care Coordinator, Case Manager, or similar role, specifically within children's services or paediatric complex care.
• Expert Knowledge: In-depth understanding of relevant legislation, frameworks, and policies (e.g., safeguarding, personalized care, relevant disability/children's acts).
• Exceptional Communication: Proven ability to communicate effectively, empathetically, and clearly with children, families, and diverse professional groups, including managing sensitive and complex conversations.
• Organisational Skills: Outstanding time management, prioritisation, and administrative skills to manage a demanding caseload and complex appointments.
• Driving: Full UK driving licence and access to a reliable vehicle for travel to family homes, schools, and meetings.
