Summary
The focus of the role will be to refer and signpost to the correct services to support early intervention and prevention of reliance on more specialist services, while reducing pressure on GP practices. Referrals will
be received from primary care health and social care professionals aligned to the practices, as well as external organisations and patient self-referrals. Individuals can be referred to the Community Link
Workers for a variety of reasons including but not limited to social issues, social isolation, housing instability, debt/financial hardship, education/employment matters and low-level mental health needs.
Referred patients may present with addiction/substance misuse/smoking alongside other issues that can impact negatively on both lifestyle behaviours and overall health and wellbeing.
A full UK Drivers license and a vehicle will be required as you will be expected to travel across several different bases.
