Summary
Core Responsibilities: The duties and responsibilities to be undertaken by the Wound Care Co-ordinator may include any or all the items in the following list. Duties may be varied from time to time under the direction of the Nurse Manager, dependent on current and evolving workload and staffing levels: Working alongside the nurse management team in co-ordinating wound care /treatment room provision. Assess and review non- healing wounds within your competency putting a plan of care in place. Act as a lead for managing complex wounds within your competency. Ensure timely referral for Doppler and manage patient waiting lists for the procedure. Facilitate effective communication between patients, members the primary health care team, secondary care, and other associated health care agencies. To provide information and advice to patients and carers about arrangement and availability of health care professionals. Refer patients on to community or secondary care when required to do so. Liaise with the Tissue Viability Nurse Refer patients to appropriate services either within or outside the practice, i.e., diabetes team, weight management. Help with safety netting plans for patients. To provide information and advice to patients and carers about arrangement and availability of health care professionals. Help with the booking of suitable follow up visits for patients liaising with family members and carers as required. Involve patients in decision making. Assist with auditing and project work in relation to wound care, within your scope and capacity. Identify and develop training opportunities for staff. To develop knowledge of services and information on local community services and maintain this knowledge through web searches, your own initiative, and other available resources. Develop and maintain contacts within all the practices in our PCN to support strategic plans. Keep electronic records of referrals and interventions to assist with audits and increase the achievement of Key Performance Indicators (KPIs). Monitor and action clinical tasks. Additional clinical skills and responsibilities: Undertake, record, and follow guidelines for the tasks for which you have received appropriate training and achieved competency Measure and record the following in routine presentations - Blood pressure and pulse - Height and weight body mass index - Urine testing - Measurement of blood glucose Assist in raising awareness and promotion of health within own competence Give accurate and appropriate information to patients within own competence Assist the patient and other clinicians in the delivery of care in urgent or emergency situations, including initiation of effective emergency care Act as chaperone and assist patients Prepare and maintain environments and equipment before, during and after patient care interventions according to Infection Control guidelines Infection control monitoring Maintain and control stock levels of clinical equipment and supplies, Physically assist in the transfer and movement of materials and equipment Assist with health promotion campaigns Maintain accurate and complete records and other documentation in accordance with Practice protocol Be aware of child protection issues and guidelines Assist with the training of other staff members and with the education of doctors and nurses in training as appropriate Work with the practice population and wider community to promote healthy living and encourage uptake of services
