Summary

Job Description Clinical Pharmacist (CP) Job Overview The Clinical Pharmacist will work as an integral member of the multidisciplinary team across the Primary Care Network (PCN) to improve patient health outcomes, particularly for those with long-term conditions. The role will focus on optimising medicines use, supporting safe and cost-effective prescribing, and enhancing communication between patients, practices, and wider healthcare providers. The postholder will provide specialist pharmaceutical expertise and evidence-based guidance to clinicians and patients, support medication safety initiatives, and contribute to the achievement of QOF and IIF targets. They will either hold, or be working towards, an Independent Prescribing qualification and undertake face-to-face consultations, delivering patient-centred care within their scope of competence. Key Responsibilities Clinical Care and Long-Term Condition Management Conduct face-to-face and remote medication reviews for patients with multiple or complex long-term conditions. Act as an Independent Prescriber (if qualified), managing long-term conditions and prescribing within professional competence. Provide holistic care by reviewing multiple conditions in a single consultation where appropriate. Support patients to understand and manage their medicines effectively, promoting adherence and self-care. Review prescribing for clinical appropriateness, safety, and cost-effectiveness; stop unnecessary or harmful medicines and implement changes directly. Review and action pathology results for patients under care. Contribute to QOF and PCN service delivery targets related to medicines optimisation and chronic disease management. Conduct domiciliary visits for housebound patients where required (subject to indemnity cover). Medicines Optimisation and Safety Lead implementation of safe, efficient repeat prescribing and medication monitoring systems. Ensure appropriate monitoring of high-risk medications and establish recall systems. Review and act on MHRA and other safety alerts promptly. Conduct clinical audits and use findings to support continuous quality improvement. Promote cost-effective prescribing in line with local formulary, NICE, and ICB guidelines. Work with the Medicines Management Team and community pharmacy colleagues to deliver PCN priorities. Provide advice on suitable alternatives during medicine shortages or unavailability. Education, Training and Professional Development Provide education and training to GPs, nurses, and other healthcare professionals on therapeutics, prescribing, and new clinical guidance. Evaluate and disseminate evidence-based updates (e.g., NICE guidance). Lead teaching sessions following clinical audits to support quality improvement. Support medical students, GP registrars, and other trainees in medicines-related learning. Participate in the CPPE Primary Care Pharmacy Education Pathway (if applicable) and maintain up-to-date professional revalidation. Medicines Reconciliation and Communication Reconcile medicines following hospital discharge, outpatient appointments, or care transitions. Identify and resolve unexplained changes, ensuring timely communication with patients and relevant providers. Produce and implement medicines care plans including monitoring and follow-up arrangements. Act as the PCN lead for communication between general practice, hospital pharmacy, community pharmacy, and care homes. Ensure care home residents receive annual medication reviews and communicate any changes clearly to staff and patients. Support initiatives to reduce medicines-related hospital admissions by identifying at-risk patients and intervening proactively. Support for Self-Limiting Conditions Provide advice and management support for patients with minor ailments (e.g., UTIs, hayfever, conjunctivitis, emergency contraception). Empower patients to manage self-limiting conditions using evidence-based resources and education. Governance, Leadership, and Systems Development Lead on implementation and review of repeat prescribing, repeat dispensing, and medicines waste reduction processes. Reauthorise repeat prescriptions within scope of practice as an Independent Prescriber. Support the practice team in responding to medicines queries from patients and administrative staff. Contribute to the development of local prescribing policies and protocols. Support safeguarding processes and ensure all incidents are reported in accordance with practice policy. General Responsibilities Maintain professional registration with the General Pharmaceutical Council (GPhC). Maintain valid indemnity cover. Engage in ongoing CPD and revalidation. Participate in annual appraisal and objective-setting. Undertake any other duties commensurate with the post, as required by the PCN.
