Are you a passionate and dedicated Social Worker experienced in adult social care? 3D Recruit is working with the London Borough of Merton to recruit a Qualified Social Worker into their Older People’s Team. Key Responsibilities:
Undertake Care Act assessments and reviews in line with statutory duties.
Complete NHS Continuing Healthcare (CHC) assessments and liaise with health partners.
Carry out safeguarding adults enquiries under Section 42 of the Care Act.
Deliver assessment and care management functions in a community setting.
Present cases to the Outcomes Forum for funding decisions.
Work collaboratively with health and social care professionals to promote independence and wellbeing.
Essential Requirements:
Recognised Social Work qualification and registration with Social Work England.
Proven experience in Care Act assessments and NHS CHC assessments.
Experience of safeguarding adults enquiries in a community setting.
Strong knowledge of adult social care legislation, policies, and practice.
Excellent assessment, care planning, and report-writing skills.
Ability to work effectively in partnership with multi-disciplinary teams.
