Care and support in Hull is transforming and Hull City Council are looking for exceptional individuals like you to join the Adult Social Care team. If you have experience of working directly with vulnerable adults, families and carers and of support planning, the eligibility determination process, social care assessments and reviewing process and/or housing legislation and supporting tenancies then we have an exciting opportunity for you.
You will have solution focused conversations enabling people to shape their own lives, identify their own outcomes and steps forward and provide interventions to sustain their independence. You will have a working knowledge of eligibility criteria for Housing and Social Care to be able to offer people options, choices and support to access re housing and care as appropriate.
