Summary
Independently assess, diagnose, and manage patients with acute and chronic conditions Conduct face-to-face consultations and home visits within scope of practice Prescribe medications and refer patients to appropriate services when necessary Manage patient caseloads, including treatment planning, referrals, admissions, and discharges Perform clinical examinations and interpret diagnostic tests Support long-term condition management and preventative care initiatives Maintain accurate and timely clinical documentation in line with practice policies Participate in clinical audits, quality improvement, and service development Contribute to safeguarding and risk management processes Collaborate with GPs, nurses, healthcare assistants, and administrative staff to ensure seamless patient care
