Summary

Job Title: Advanced Nurse Practitioner (ANP) Location: Oak Lodge Medical Centre Reports to: Line manager and clinical lead Salary Band: 58-63K depending on experience Contract Type: Full-time/Part-time, Permanent Job Summary We are seeking a highly skilled and autonomous Advanced Nurse Practitioner (ANP) to join our multidisciplinary team. The postholder will provide expert clinical care, including assessment, diagnosis, treatment, and management of patients across a broad spectrum of acute and long-term conditions. The ANP will contribute to clinical leadership, practice and PCN development, and the delivery of high-quality, patient-centred care. Key Responsibilities 1. Clinical Duties Independently assess, diagnose, and manage patients presenting with undifferentiated and minor illnesses (e.g., respiratory infections, urinary tract infections, skin conditions). Provide expert management of long-term conditions, including: Diabetes Asthma Chronic Obstructive Pulmonary Disease (COPD) Hypertension Heart Failure Conduct medication reviews and prescribe safely and effectively in line with current evidence-based guidelines. Request and interpret diagnostic tests such as blood investigations, ECGs, and X-rays. Deliver health promotion advice and support patients in self-management and lifestyle modification. Triage and manage same-day urgent presentations appropriately and efficiently. Refer patients to secondary care, community services, or other appropriate agencies where necessary. 2. Practice Development Contribute to the development, review, and implementation of clinical guidelines, protocols, and care pathways. Actively support the achievement of QOF (Quality and Outcomes Framework) targets and other locally commissioned services. Participate in clinical audit, service evaluation, and continuous quality improvement activities. Engage in practice meetings, MDT discussions, and strategic planning initiatives. 3. Leadership and Mentorship Provide clinical leadership, support, and supervision to nursing and healthcare support staff. Contribute to the induction, teaching, and ongoing professional development of new staff and students. Act as a role model for evidence-based practice and lifelong learning. Champion a culture of safety, collaboration, and accountability within the clinical team. 4. Governance and Safety Maintain accurate, legible, and timely patient records in line with professional and legal standards. Comply with safeguarding protocols and contribute to the protection of vulnerable patients. Uphold high standards of infection prevention and control, confidentiality, and data protection (including GDPR compliance). Work within the scope of professional practice and in accordance with NMC code and relevant legislation. Essential Criteria Registered Nurse with the NMC MSc in Advanced Clinical Practice (or working towards) Independent Prescriber (V300) Proven experience in autonomous clinical practice Advanced clinical assessment and diagnostic skills Experience in managing long-term conditions and minor illnesses Excellent communication, leadership, and decision-making skills Desirable Criteria Experience in primary care or general practice setting Experience in clinical audit or quality improvement initiatives Teaching or mentorship qualification Familiarity with EMIS clinical systems Working Conditions The role involves face-to-face, telephone, and digital consultations. Occasional home visits may be required. The postholder will be expected to participate in extended hours or weekend clinics as needed.
