Summary

Job Summary: The post holder will play a pivotal role in providing advanced nursing care to patients with long term chronic diseases, ensuring they receive comprehensive support, education and management tailored to their individual needs as well as the provision of preventative health care to the practice population. Job responsibilities: Deliver specialist diabetes care, assessment and treatment to our diabetic patients, including initiation of insulin therapy. Work with other health care professionals to diagnose, manage and monitor long term conditions, including non-drug based treatment methods using a management plan and in line with National and Local policies and practice needs. Assess, diagnose,plan implement and evaluate treatment/intervention that promotes health and wellbeing, as well as care for patients presenting with an undifferentiated diagnosis. Clinically examine and assess patient need from a physiological and psychological perspective and plan clinical care accordingly. Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan. Prescribe, review and instigate medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, NICE guidance and practice protocols. Work with patients in order to support compliance and adherence to prescribed treatments. Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self care. Advise, support and where appropriate administer vaccines for patients travelling abroad. Implement and participate in vaccination and immunisation programmes for both adults and children. Able to recognise and manage anaphylaxis according to current UK guidelines. Duties will vary from time to time under the direction of the Partners/Practice Managers dependent on current and evolving practice workload and staffing levels.
