Summary

To identify administrative and professional needs that support the team and the post holders own objectives via regular meetings with both the Senior Physical Activity Specialist/physiotherapist and the Lead Nurse for CR This is achieved through Appraisal and your Personal Development Plan (PDP). To keep up to date with knowledge and practice, by attending relevant post-related courses. To maintain BACPR phase IV exercise instructor status. Teaching and training To provide education to patients and carers relevant to the program through adopting healthy lifestyle. Deliver educational topics using a variety of mediums including face to face, group sessions and individualised sessions. Clinical governance To be responsible for keeping safe patient information such as record cards when in transit in accordance with information Governance. To be responsible for ensuring information (paper-based, verbal, or computerised) provided to clients or other health professionals is up-to-date, evidence-based and appropriately delivered. To comply with the Trust and department operational policy on all documentation whether computerised or paper-based Ensure that resources are monitored and utilised in accordance with the CR communication budget. Research Ensure all physical activity data is accurately collected and collated using the CR documentation and database system to contribute to the evaluation of the programme. Ensure any information/data required for clinical audit or statistical analysis for research projects or registers is recorded and collected as appropriate. To liaise with CR directly and monitor/evaluate progress and achievements on the outputs. Participate in local and/or national research and development initiatives as appropriate. Other duties To work with the CR team to develop the service. Liaise with and provide support to all other members of the CR multidisciplinary team and attend team and management group meetings as appropriate. Responsible for the economic and appropriate use of apparatus, for example exercise equipment, including setting up and dismantling display equipment safely. Dealing sensitively with patient/carers who may be distressed. Conducting assessments and treatment at a variety of settings throughout the day Simultaneous management of different aspects of the job description There may be a requirement to work with people who have barriers to their understanding of key messages, for example those whose first language is not English. There may be a requirement to deliver informal presentations to community groups or groups of staff. Your healthcare closer to home To produce, distribute and disseminate publicity and promotional material to groups and areas targeted in the Borough as part of a planned marketing strategy. To assist in promoting an understanding of the CR and its work To assist in health checks in a community setting To be flexible with location and time of working to ensure accessibility of the programs to all. This may include working on different sites or in the evenings or at weekends. To support local patient support groups, or in their absence, facilitate the development of such groups, to facilitate user involvement when producing relevant work. To continue to support the local Heart Support Group. To maintain close links with Phase IV (community) exercise instructors. To undertake any ad hoc duties necessary to deliver the service. To support other services within the team when the need arises.
