Summary
In this role, outreach workers will:
• Provide one-to-one, trauma informed support in the clients home or local community, coproducing a support plan with the client based on their goals and aspirations.
• Co-facilitate a small number of support groups (e.g. bereavement) and pilot your own ideas for support groups, workshops or wellbeing sessions
• Help people build resilience, improve wellbeing, and connect with their community
• Advocate for those facing barriers like housing, income, and social isolation

Required
• Occasional evening slots for clients who are working
