Summary


Bolton Hospital Foundation Trust are looking for a Diabetes and Endocrine Specialist Nurse, Band 7 for a full time post. This is a unique post where the successful candidate would work full time split over 2 specialities, working 2.5 days as a Diabetes specialist Nurse and 2.5 days as a Endocrine specialist nurse. Band 6 - 7 development would be considered for a suitable candidate.



Toworkasanautonomous,advancedpractitionerandeffectivelydeliveranexpert servicewithinthespecialistareabothfromaninpatientandoutpatientbase.

TocontributetoahighqualityintegratedDiabetesandEndocrineservicethatprovides careforpatientswhohaveadiagnosisofDiabetesorestablishedEndocrineconditionswithinprimary,secondaryandspecialistcare.

Toprovidespecialistcaretopatientsensuringboththepatientandtheirfamiliesarefullyinformedandsupported.

To act as a resource and educator for both patients and staff in a hospital and communitysetting,providingadviceandknowledgeinmanagingthesehighriskgroupsofpatients.

Maintain working relationships with the DiabetesandEndocrine MDT includingConsultants,SpecialistNurses,Podiatrists,Dietitians,Supportstaffandclericalteams.
