Camden Rapid Access Services (RAS) is aPhysical healthadmission avoidance and supported early discharge service. Which has three pathways Discharge to Assess, Urgent community Response and Virtual ward, the potential post holder will work across all three pathways within Camden RAS.
The potential post holder should ideally possess a valid UK Driving licence (a car will be provided when required), though suitable non-driving candidates will be considered and provided with an Oyster card while on shift. Non-driving practitioners will be expected to be able to undertake the same number of visits per day as driving colleagues.
Are you a qualified advanced clinical practitioner (ACP) seeking a new challenging role in the community? The Camden Rapid Access Services (Camden RAS) is currently seeking to employ a Band 8a Registered ACP with substantial experience in acute care (e.g. A&E, AMU, ITU, Virtual Ward, Paramedic). The ideal candidate will be a non medical prescriber V300 with advanced clinical assessment skills, and have completed the ACP modules.
Our overall aim is to avoid unnecessary hospital admissions and early supported discharge, as part of the NHS Long term plan. The benefits of keeping patients in their own homes are numerous and varied, from reducing Healthcare Acquired Infections, to promoting ongoing independence.
Shifts
The shift pattern will be Monday to Friday 9am to 5pm, which may change in the future if we move to a seven day service. The Trust also offers flexible working opportunities.
Lone Working
This role regularly involves working independently in patients’ homes or within the community, including during evening hours when it may be dark. Please be assured that robust safety systems are in place, and we maintain an excellent safety record.

We are seeking a dedicated Advanced Clinical Practitioner to join our team, sharing our commitment to delivering exceptional quality care and developing comprehensive treatment plans for service users in their homes. As a key member of our multidisciplinary team, the post holder will collaborate closely with consultants, GPs, pharmacists, nurses, therapists, and healthcare support workers.
The role requires a high level of autonomy and complex decision-making, supported by a master's level qualification encompassing clinical practice, leadership, management, education, and research. The post holder will manage clinical care in partnership with individuals, families, and carers, analysing complex problems to devise innovative solutions that enhance outcomes and experiences.
Key responsibilities include delivering advanced clinical practice autonomously in general medicine, frailty, respiratory, and cardiology, focusing on supporting patients as an alternative to hospital admission and early supported discharge. Collaborating with colleagues and managers, the post holder will integrate care pathways and patient management across community and acute settings.
The role emphasizes the delivery of consistent, high-quality, person-centered care through advanced clinical judgment and critical thinking, aligning with the National Framework for Advanced Clinical Practice and embracing the four pillars of clinical practice: leadership and management, education, and research.
